About the Role

Are you passionate about making a positive impact on the health and well-being of children and families? We have an exciting opportunity for a Community Health Worker with one of our medical clinics.
Schedule: Monday - Friday (NO WEEKENDS)
Payrate: $20 (DOE)
Responsibilities:
- Help patients and families access community resources.
- Collaborate with our healthcare team.
- Track referrals and provide follow-up.
- Assist with data reporting and policy development.
Qualifications:
- CHW certification (or obtainable within 6 months).
- High School Diploma or GED.
- 2-3 years of community involvement (strong ties to the community is a plus).
- Basic Life Support (BLS) certification.
- Strong communication and organizational skills.
